The Alberta Heartland Primary Care Network (PCN), serves Fort Saskatchewan, Gibbons, Lamont, Redwater, and the surrounding area. It includes a group of 30 physicians in 15 clinics are working with a multidisciplinary team of health professionals to provide care for patients and to coordinate primary health care services. Their team includes registered nurses, registered dietitians, an exercise specialist, a pharmacist, registered psychologists, a social worker, nurse practitioner, an occupational therapist, specialist referral coordinators, and administrative staff.
